I have heard a lot of good things about Kaiser Permanente health care. For the last few months I have been doing some researching and reading up on a few things for my mom as well as myself, and time and again I come across people praising Kaiser and their willingness to just process claims without denying them or requesting that their clients jump through hoops first.
